My name isn't showing up correctly, how can I change this?

If your name is not showing up correctly on the Online Register, please email us at registration@physioboard.org.nz with the following information:


  • If you name has not changed and is just listed incorrectly, we need a copy of your passport or government issued ID,
  • If you name has changed, we need evidence of name change e.g. marriage certificate.
  • We have 3 fields in our database for names. When generated on an APC it reads as First Middle Last. Confirm what parts of your name need to be in which section:
    • First
    • Middle
    • Last
